The Case Manager representative is the patient’s
advocate and provides the following services:
- Information and assistance with completion of
Directives to Physicians (living wills)
- Information and assistance with completion of a
Medical Power of Attorney
- Assistance with obtaining medical equipment
- Referral to for Hospice Care.
- Referral to nursing homes and assisted living
facilities
- Assistance with obtaining financial aid (i.e.,
Social Security Disability, SSI, etc.)
- Referral to Swing Bed and other rehabilitation
services
- Referral to the Home Health Agency of patient’s
choice
- Facilitate transfers to other hospitals or medical
facilities.
- Assistance with any other Social Services or Ethical
issues.
