Case Managers
The Case Manager representative is the patient’s advocate and provides the following services:
Information and assistance with completion of Directives to Physicians (living wills)
Information and assistance with completion of a Medical Power of Attorney
Assistance with obtaining medical equipment
Referral to for Hospice Care.
Referral to nursing homes and assisted living facilities
Assistance with obtaining financial aid (i.e., Social Security Disability, SSI, etc.)
Referral to Swing Bed and other rehabilitation services
Referral to the Home Health Agency of patient’s choice
Facilitate transfers to other hospitals or medical facilities.
Assistance with any other Social Services or Ethical issues.
